Optical

Our Optometrists at Crusader Community Health provide a complete comprehensive eye examination including dilation. They treat eye infections, remove foreign bodies, evaluate Diabetic patients, give prescriptions for glasses and provide other primary eye care services such as screenings for diagnosis of cataracts and glaucoma.

All services are provided by Illinois-licensed optometrists. In addition, the department operates an optical dispensary for corrective lenses and eyeglass frames. Optical equipment includes an auto-refractor, a non-contact tachometer and instruments to make it easier to deliver services to those confined to wheelchairs.

Optical Shop
The optical department offers a variety of frames and lenses to serve your optical needs including lined bifocals, no-line bifocals, fashion tints, transition tints, plastic and lighter weight thinner lenses. Frames and lenses start at $45 complete.

Monday through Friday – 9 a.m. to 5 p.m.
Closed during lunch from Noon to 1 p.m.

Please call at 9 in the morning of the day in which you would like to be seen for an eye exam and we will try to schedule you for an appointment that same afternoon.

Payment Policy
*Medicaid will cover one eye exam, Medicaid approved frame, and one pair of single vision or lined bi-focal lenses per year.  For your convenience, we will bill Medicare.
 
Patients without Medicare or Medicaid, may be seen on a sliding fee scale. Patients are eligible for 25%, 50%, 75%, and 100% discount after a $30 co-payment. The discount is based on their gross income and does not apply to eyewear.
