Mission: Crusader Community Health serves the Rock River Valley area with quality primary health care for all people in need.

Location: Only CHC in Northwest Illinois; 2 locations in Rockford-West State & Broadway; Belvidere; Freeport (pediatric dental); Crusader Community Health Loves Park - opening September 2011

  • Over 90% of patients surveyed in 2010 indicated they would refer a friend or family member to Crusader
  • Accredited by the Joint Commission 4 times since 1999
